It is my understanding that a Buy It Now purchase is up for grabs. However a buyer can take advantage of Auctions and Accepted Best Offers to delay payment for as long as possible is what I understand? Therefore if my auto-accept offer triggers, that doesn't mean I get paid until the buyer actually pays vs flat buy it now

To the best of my knowledge, a buyer has 4 days to pay (unless it's listed with IPR). If that has not happened by day #4, the seller can cancel, citing "buyer did not pay" as the reason. I believe that applies to accepted best offers as well as any other regular situation.

if it requires Immediate payment and you don't pay I do think someone else can come along and buy it.
If there is no immediate payment required it is yours and you have 4 days to pay.

"Immediate payment" only applies when purchased at full price Buy it Now only.
If you add offer to a buy it now listing for a listing that requires immediate payment, the buyer does not have to pay immediately if the offer is accepted, even automatically.

Also, even with BIN/Fixed Price you need to tick that option in your requirements - Immediate Payment Required.
In the case of auctions and offers:
If a buyer hasn’t paid within 4 days, on the 5th day you can go to your Seller Hub to cancel the transaction using the “Buyer hasn't paid” reason and this gives the buyer a strike and you can relist. In future to have eBay automatically cancel your transactions due to non-payment:

and set your buyer requirements to 2 UPI strikes in 12 months.
