Are you shipping direct or through 'eBay International Shipping'?
If you're shipping direct Int, then you can combine the items as you see fit.
If you're shipping through eIS the items should combine automatically or suggest combining before label purchase. If they do not then they must in fact be shipped separately.

With EIS, you can combine shipments, even if the transactions went through separately. eBay doesn’t make it easy. I’ve done it a couple of times – made me nervous each time. What I did was create one shipping label, added a couple of pieces of paper to the box that indicated all of the order/shipment numbers. Then, I wrote every single order number/shipment number on the box. I don’t mean the order number that you get that takes you to the details, I’m talking about that unique identifier that goes on each and every EIS shipping label. Done it twice – worked twice.
