How do you close out an order after issuing a full refund and shipping the item, without initiating a formal cancellation?

What? You refunded an order but shipped it anyway? Why?
Typically if you refund in full that tends to be treated as an out of stock cancellation which gives you a defect against your account. Now shipping a refunded order, that’s a new one on me.
