Buyer offer accepted no payment processed

I have had 3 buyer offers for items today, accepted and then disappear. My business policies require immediate payment. Why is this suddenly started to happen and 3 different occurrences today. Has a new process been implemented or a glitch in payment occurring? I dont want to lose orders so please advice.

We have seen this happen when a buyer makes a counteroffer on an automated offer you sent.
