Would you refund in this situation?
I recently sold 2 similar items on the same day. I don't know how I did it, because I even wrote on the shipping label when I printed it out what it was for, but I got the labels switched and sent the wrong items to the wrong people. Was horrified when I found out. I immediately contacted both buyers, we all agreed that I would send them pre-paid shipping labels (paid for by me) and they would switch packages. Everything went off without a hitch. They just had to print out the label, put it on the package, and stick it in their mailbox. Now one of them wants me to refund them the amount of the INITIAL shipping for her trouble. Instead of putting the package in her mailbox, she drove it to the post office and says she deserves the money back for her trouble.

-Well, it was your mistake so I would refund her and move on
-When I switched labels I had both Buyer return the items to me and I refunded their out of pocket expense off their original purchase. Once I had both items back I inspected them and paid for shipping again to the correct Buyers. My mistake, I paid for it. Profit on those two transactions obviously isn't worth talking about LOL but for me I did the right thing.
