Where can I get a copy of Ebay's W9?

The W9 form is a request for taxpayer ID - it is a form eBay sends to members when the IRS has indicated that the supplied SSN or EIN does not match their records.
If eBay has sent you a W9, you should also see a banner on your account settings page with a link to enter the info online instead of sending in the form.
