Each item is an insertion. There is no additional charge for putting a buy it now price on an auction. Each item that only has a buy it now price - a fixed price item - uses an insertion each month it is active.
How many insertions you get for no additional charge depends on your store subscription level. A starter store gets 250 total insertions. A basic store get 250 auctions in certain categories plus 1,000 fixed price items, more in certain categories.
