Returns
Do you have an option when buyer states the item is not as pictured when it is exactly as pictured and they want to return it for that reason? Does this put a ding on your feedback?

If you do not accept the return, and eBay has to step in, they will say return, or worse - refund from your funds without return. When eBay has to step in, you get a ding on your performance (defect) and the fees are not refunded.
Only the buyer can leave less than positive feedback. I think you can expect this, if you force them to ask eBay to step in.
You offer returns, so the only issue is the reason selected by the buyer. False returns are part of selling online. Accept the return, refund when received, block the buyer and move on to things more worthy of your time.
Since you are top rated, if you were to make your 14 day return policy seller pays, you would get a 10% discount on your final value fees. If you don't get many returns, this could more than offset the cost of shipping on the false returns.
