return from usa to uruguay, how?

In Uruguay, we have a problem receiving shipments from abroad. We can only receive 3 shipments per year tax-free (60%), and they are valued under US$200.
That's why I list my items as "I DO NOT ACCEPT RETURNS" and I try to include clear photos and concise descriptions to avoid problems.
I'm not an amateur; I've been selling through eBay since March 2001, and I have more than 11 positive feedback ratings (100%) and 26,000 items sold.
Now the problem: I sold a soccer jersey with all the details, measurements, etc., and I even took photos of it when I measured it, precisely to avoid this problem.
A buyer (I assume he's new, given his 26 reviews) has purchased a product and wants to return it (according to him, it's different from the listing...).
Despite not accepting returns, in this case (I still have a deductible that I haven't used yet to bring items from abroad), I told him it was no problem, to send it and I'd refund his money.
He also doesn't want to pay for shipping (you can't pay for the USPS label from Uruguay).
I still haven't clicked "accept return" or "do not accept return." I feel frustrated because I already feel distrustful. The system only protects buyers, leaving sellers (especially those from outside the US) unprotected, as they treat us with the same standards as those who live in the US.
I really don't know what to do. I don't get any answers from customer service, and the information I find is all in English (I'm currently using an online translator).
I feel extremely frustrated.
