Item that was refunded was just received over a month late
I paid for an item that I did not receive and I had no communication from the seller after multiple attempts to contact them. I then put in a request for a refund and after the appropriate time it was refunded. Today over a month later, I received the package. Do I pay the seller for the item?

If you still want the item, you should pay the seller for it. You and the seller can agree on a payment method - PayPal, venmo, zelle, personal check - whatever works for you.
If you no longer want the item, ask the seller to send you a return shipping label.
