Item Does ot Match Description Returns

I bought an item which does not match the description. The seller said it was the same, which I believed initially.
After a stroke, my wife could use easily the described item, but not the bought item. It's geometry was different and not usable for us.
It was more than 30 days, but less than 90 days as per ebay guarantee. I contacted seller and they would not do a refund. Ebay refund systems seem to accommodate within 30 days only.
What can I do?
