How to stop buyers registered in South Africa but have a mailing address in the U.S. from buying
In the last two days, I have had 2 different buyers purchase silver coins from me that are registered in South Africa. I have ALL countries blocked from buying except the United States (lower 48). My question: How can someone registered in South Africa but with a mailing address in the United States able to purchase my items? Is it safe to assume Ebay uses the mailing address instead of registration location in determining who can buy items from me?

You misunderstand. eBay does not allow any seller to block any "countries" from buying.
eBay allows sellers to block shipping addresses in countries other than the USA. As you know, you are also allowed to block shipping to Alaska, Hawaii, Puerto Rico, APO and FPO addresses (military and navy), etc.
"My question: How can someone registered in South Africa but with a mailing address in the United States able to purchase my items?"
Well, if he/she has the money to pay for what he/she wants and has a mailing address in the United States,
why would he/she not be able to purchase your items?
"Is it safe to assume Ebay uses the mailing address instead of registration location in determining who can buy items from me?"
Yes, that is precisely what happens. And precisely because: "eBay does not allow any seller to block any "countries" from buying."
On eBay.com, a seller cannot validly stop a sale to any buyer who has a mailing address in the U.S.
If a seller cancels such a sale, eBay will give a "selling defect" to that seller. Enough defects and the seller's eBay days will be numbered.
