Go to your Purchase History and find the link out to the right of the listing. If it isn't evident, check under the More Actions button.
If there isn't a link on your PH page, then go to your feedback page. Scroll to the bottom of the page, and click the "Leave Feedback" link down there.
You get 60 days to leave feedback. If you can't find the link in either place, the most likely reason is that it's been too long and the opportunity has expired.
