Help shipping from US to Brazil - customs form?

Question

I am not sure what I need to fill out on the customs form to make sure I have all the info I need. Does anyone have experience with this who can help? Thank you so much!

@racmcm2181 What is the item number you are trying to ship? Not all items can ship to all countries due to import restrictions.

I see you are direct shipping some items and others you are using the eBay International Shipping Program. I would recommend sticking with the eBay International Shipping Program. It has to be on the listing before the buyer purchases though.
