What do you do when you sent an item it has been tracking for over 2 weeks but buyer never received. I am obliged to refund buyer or is it handled a different way?

There are 2 other options on the USPS site to find the package. If the buyer hasn't opened a case yet and is just enquiring, be proactive. Work with the USPS and let the buyer know. If the buyer has opened a case, you do have to refund.
As a seller, I am always proactive. I watch tracking on every item and if something is running late, I message the buyer. I ask for email updates and then send those to the buyer. If something requires pick up, I let the buyer know when it is available at their local PO.
