Sellers are not required to combine shipping and often times it is impossible. There are multiple things to take into account: weight of the items, dimensions, shipping type (media/priority/ground), types of items, etc. There are times when it is impossible to combine shipping. Always ask first rather than assuming.

@omgodlike ,
Is this happening when you send multiple offers or bid on multiple auctions from a seller?
Many sellers who auction items or allow best offers, require immediate payment aka Auto Pay, which indicated by a note saying "if the offer is accepted or You win the auction, your payment source will be charged". when you send an offer or place a bid.
Since those payments are taken automatically the seller cannot send a combined shipping invoice. Some will combine the items and send a refund for the shipping overage.
Some of those sellers may not know they have been opted into the Auto Pay system by ebay.
There are sellers who do give instructions on how to get a combined shipping invoice on their listing pages, and if you follow them they can send you a combined shipping invoice. Usually it involves you adding the items to your Cart without paying, then request a combined shipping invoice.
Because it is up to the Seller on whether or not they will combine and reduce shipping costs. On items requiring immediate payment they would have to be combined AFTER they were purchased and then the Seller refund the shipping overage.
Some problems we see here is some Sellers just don't understand the shipping end of online selling. They make mistakes before figuring it out. Some Sellers use shipping as an additional profit center so they can display a lower item price on their listings.
eBay does no control what a Seller does or doesn't do in the area of Combined shipping. You have the option to message a Seller and ask them how they handle Combined shipping and see how or if they respond.
Yeah its kind of a dumb option sent a message and 10 mins later got the invoice with no change shrug =o and now I have to buy the items with no discount

Really sorry that happened. For myself I view shipping as a break-even proposition. On multiple purchases I refund the overage and on auctions if the Buyer doesn't request an updated invoice I send one with the combined and reduced shipping cost on it. eBay charges their fees on what the Buyer pays for the item, their paid shipping cost and sales tax. Keeping the shipping cost down actually cuts down on fees paid to eBay.
