There is no guarantee on a Seller's legitimacy but the vast majority of them ARE legit. You can also message the Seller and ask questions about the item, serious ones of course not just questions to see if anyone is home.
If you purchase a Local pick-up item through eBay (opposed to paying cash at pick up) you are protected by the eBay Money Back Guarantee for 30 days: eBay Money Back Guarantee policy
However, in the case of a return you would have to take it back to the Seller.
On Local pick-up transactions eBay allows personal information to flow back and forth between Buyer and seller for the purpose of setting up the day, time and location of the pick up. I do several per year and use a local convenience store, it is an easy landmark to find and if the Buyers need gas, snacks, restrooms, etc. its all right there for them.
