buyer says item wasn't received tracking says delivered, they are asking for a refund, do I refund? Do I trust them and refund the money and then am out the money and the item?

If the buyer files am Item Not Received case, you respond by entering the tracking number that shows Delivered. ebay should then find in your favor.

@3so9032 < Do I trust them and refund the money > No. There might be some kind of problem, but it isn't yours. The package could have been misdelivered. It could have been stolen off their porch. And those explanations assume the buyer is telling the truth. He could be trying his luck at ripping you off. He might try this on every seller. There's just no way to know the rest of the story.
