Buyer lies about an item just to get a free return label will I get a refund?

After accepting the return and issuing the refund when received, report the buyer for abusing the money back guarantee. Since you are top rated, eBay will subsidize $6 of the return shipping, remove any bad feedback from the transaction, and remove the return from your service metrics.
They will never tell you what action is taken against the buyer, but we do get buyers who come here and whine about no longer being able to request a return.
