How does Ebay protect a seller from a buyer who keeps the original item and sends in a lesser valued item for a refund?

Report the buyer. There is a blue "Report a buyer" button on the help page below:
Example: "You believe the buyer is misusing the eBay Money Back Guarantee or the returns system"
