The ask for invoice button or ask for combined shipping button is always missing for my customers. I have already switched away from requiring immediate payment as ebay suggested. Even when I sign in as a guest I can't see a button for my store in pre-checkout. I'm losing sales because buyers don't trust I'll send a refund.

If you have turned Combined shipping ON have you checked to make sure the Auto-pay is turned OFF on this page near the bottom? There is a setting for both Offers and Auctions: Buyer management | eBay
