If I sale an item that is Size medium and still has tags (new) stating that it's a size medium but the buyer receives the item and says no way this is a medium and wants a refund , what should I do? I feel like I can only go by what the tag says

Ebay wants you to accept the return, send a return label, and refund when you got the item back.

If you don't, the buyer can ask eBay to step in. Ebay can refund the buyer without requiring the buyer to return the item. And you will receive a nasty defect.

Accept within 3 business days.

Also, for the future: give a size and the measurements. This can help to prevent issues.
