I know I've been through this before, but what's the process when my item I sent arrives broken. Do I issue a refund and then deal with the Post Office? Or does my buyer start the process?

I sold an item and it arrived broken. I had it insured and offered the lady a full refund. She didn't reply but left me negative feedback, not even giving me a chance to make it right with her.
A customer left A++++ feedback in the remarks but marked "negative" for feedback. He said he did it by mistake as he was very pleased with the item. Now he doesn't know how to change it to positive. Any suggestions??
Listed a LARGE item for PICK UP ONLY in Nebraska. Someone from California purchased it and paid for it through PayPal. I've tried and tried to contact them with no response whatsoever!!! What do I do?? I contacted eBay and they said to give it a coup...
